Here are the directions how to get there from Biomedicum.

Head down Podwale Staromiejskie, NFZ office will be on your left

As you enter, go to your right - Office #2 is where you can pick-up your EHIC

Keep going - office #3 is where you'll pick-up your NFZ booklet

The office you want is UMOWY INDYWIDUALNE, next to #4

At the Gdańsk NFZ, the office you need is labeled UMOWY INDYWIDUALNE, located on the ground floor, to the right of the door labeled #4.
The employees there are very polite.
They will help you fill out the paperwork and will give you all the info you need on how to make payments.
